at the hospital是泛指在医院，at hopital是生病在医院，这样理解对不？

你很不错嘛。
Bill should say: Why are you at the hospital? 不能省略这个定冠词。He is in hospital. 有他住院的意思。He is in the hospital, 是指他身在医院，不是电影院。是病人还是串门我们不知道。
